Information You Need to Gather Before Applying for Insurance
For every member of your household who needs coverage, you must provide:
- Social Security Number (or legal immigrant document number)
- Employer and income information (for example, from pay stubs or W-2 forms)
- Policy numbers for current health insurance plans
- Information on job-based plans that you or anyone in your household is eligible for (even if not enrolled in).
